For our patients with dental insurance, our professional services are rendered to you as a courtesy, not your insurance company. Insurance is a contract between your employer, the insurance company and you, our patient. Therefore, you are directly responsible to us for the obligation of payment should your insurance carrier not pay us within 45 days of receipt. California State Law indicated that an insurance company must make payment or reply to you or our office, for services within 30 days. We will do our utmost to help you receive the maximum insurance benefit available. We will accept assignment of benefit from your insurance company. At the time of your appointment, we do request that you pay in full any estimated uncovered portion of your care and deductible. The percentage of coverage by your insurance company maybe based on the insurance company’s own reduced fee schedule for dental services and maybe less than actual charges resulting in a lower coverage. This lower payment is a direct result of the plan selected by your employer. Please keep in mind that some insurance companies are no longer including preventative x-rays under the preventative portion of the policy and a deductible and co-payment may apply. In addition, basic routine composite restorative fillings may also be reduced to an amalgam co-payment; resulting in an additional co-payment for yourself. We will do our best to keep you informed, but please keep in mind that providing a benefit book to our office, provided by your insurance company or employer will answer most of these questions, verses erroneous information that could be provided by an insurance company. We realize that emergencies can occur; should an unforeseen situation prevent you from making a pre-arrangement payment, please contact our office to avoid the possibility of a misunderstanding. |
